At least 1 week prior to its availment except on emergency cases, where an employee may be allowed to apply/file leave thru traditional way using the old leave form. In case an employee fails to satisfy the time element in filing/applying thereof, he/she may also be allowed to file leave thru traditional way using the old leave form, provided an Explanation-Letter noted by the Chief of Office/Hospital should be attached.

Forced | Vacation | Union/CNA | CTO

At least five (5) calendar days in advance of the effective date, otherwise the employee may be allowed to apply/file leave thru traditional way using the old leave form, provided an Explanation-Letter noted by the Chief of Office/Hospital should be attached.

Sick

Within three (3) days upon employee's return from such leave, otherwise the employee may be allowed to apply/file leave thru traditional way using the old leave form, provided an Explanation-Letter noted by the Chief of Office/Hospital should be attached.

Maternity

Immediately BEFORE, [DURING] or AFTER the actual period of delivery/miscarriage or at the date of adoption of child.

Paternity

Immediately BEFORE, [DURING] or AFTER the actual period of childbirth or miscarriage or at the date of adoption of child.

Parental

Not specified in the RA, therefore anytime within the year.
